Benefits Checklist
TSU Federal Benefits Application – Hazlewood & Residency Requirements
Students applying for federal benefits at Texas Southern University must complete the benefits packet, which includes the Residency Questionnaire, Hazlewood Document Checklist, and Semester Benefits Form. First-time applicants must submit the Hazlewood application with all required documents, while continuing applicants must complete the renewal packet each semester. All forms must be submitted electronically, and transcripts must be emailed to admissions@tsu.edu to prevent processing delays.

If you are applying for federal benefits please complete the benefits packet below, once completed you will receive a copy for your records and the veterans office will receive one for processing.

If you are applying for Hazlewood benefits as a first-time applicant please complete the first-time applicant packet and submit all the required paperwork. if you are applying as a continuing applicant please complete the continuing applicant packet and submit all the required paperwork.

Semester Benefits Form
The semester benefits form must be completed for each semester you are requesting benefits. All transcripts must be submitted to admissions@tsu.edu. Failure to submit all necessary documents will result in a longer processing time.
All forms are fillable and will automatically be sent to veteransaffairs@tsu.edu for processing upon completion. If you forget to attach any documents with your packet, please email them with your Name and T-number in the subject line.
