Surveys - Quality Enhancement Plan
Appendix 2
Freshman Student Survey
The Quality Enhancement Plan Committee is conducting a survey of first-time freshmen at Texas Southern University (TSU). The purpose of this survey is to learn about your experience at TSU and to identify ways we can help increase your success through the remainder of your undergraduate education. This survey is completely voluntary, and will only take about 20 minutes of your time. You may elect not to participate, skip any question, or stop at any time.
There are no foreseeable mental, emotional, physical, legal, or social consequences for completing the survey. Likewise, there are no penalties or benefits directly associated with survey completion. Though your responses will be linked to your T-number, instructors will not have access to your responses.
In addition, all data findings will be reported in aggregate (group) form, and will be used to inform TSU’s administration about challenges faced by first-time freshmen students as a whole. By moving to the next page of this survey, you are indicating that you are 18 years of age or older, and that you have read and understand your rights as a survey participant.
If you have questions about this survey, please contact Dr. Gregory Maddox, Chair of the TSU Quality Enhancement Plan Committee at 713-313-7889 or the TSU Office of Institutional Research at 713-313-4301.
